Job Description
The Triage RN reports to Associate Clinical Director. He/she is responsible for triaging patient and caregiver telephone calls, assisting in the solution, palliation of symptoms and provision of education. Assists with identifying patient/family care needs and for providing nursing and supportive care for the terminally ill patient and their families, in conjunction with case managers and physicians. Maintains relationships with families and patients. Provides support to care team members by acting as a liaison/coordinator with physician offices, insurance case managers, pharmacies and DME company.

Education:
Diploma, ADN and/or BSN from an accredited institution
Licensures/Certifications:
Current Maryland State license as a Registered Nurse
Experience:
One to two years of nursing. Home Health Hospice or triage experience required

* Skill in the use of EMR (Electronic Medical Record) and/or ability to learn

* Ability to simultaneously handle multiple assignments and projects with attention to details, efficiency and accuracy and deadlines.

Principal Duties and Responsibilities:
* Triages all patient and caregiver calls, assisting in the solutions, palliation of symptoms and provision of education.
* Serves as a liaison with physician offices on behalf of the care team.
* Provides immediate response and emotional support to the family at the time of death call.
* May assist in the coordination of changes in patient level of care.
* May communicate with insurance case managers on behalf of the care team. Documents discussions in detail.
* Assists with pharmacy issues (ordering medication refills, securing scripts for controlled substances, problem solves breakdown in systems communications).
* Orders supplies and durable medical equipment as appropriate and documents same in the EMR
* Coordinates equipment pick up with vendor representative as necessary.
* Trends calls prospectively to determine patterns by team/provider and identifies opportunities for improvement.
* Deploys staff as needed to address patient and family assessed needs.
